| The burn involves the whole back, whole chest, whole face, or both of whole legs (except for sunburn). | The patient has difficulty of breathing. |
| The patient has dizziness of gait instability, or he/she cannot ambulate. | The patient looks lethargic. |
| The patient inhaled smoke or flame. | The patient has throat pain OR the patient has voice changes. |
| I see soot on the patient's face. The hairs of the nostrils are burned. OR I see soot in the patient's mouth. | The patient has difficulty of breathing. |
| The patient has a racing heart. | The burn goes round the full circumference of the neck, arm, or leg (except for sunburn). |
