- Age
- Allergy (in an adult, in a child)
- Do you have a mild rash or itchness?
- Older than 65? Younger than 6?Pregnant?
If you can't find a hospital immediately, or if you have one of the following symptoms, call an ambulance.

Do you have difficulty breathing? | Do you have swelling of your mouth or tongue? |
Do you have difficulty swallowing? | Did you lose consciousness? Are you dizzy? |
Do you have chest pain? | Do you have rapidly progressing symptoms (regardless of the type of symptoms)? |
Have you ever had an anaphylaxis from the same allergen before? | Do you have a continous nausea, vomiting, or diarrhea? |
Any vision changes? | Does the patient look confused (agitated or weired response to verbal communication)? |
Do you have a swelling of your face or limbs? |
